Because the main purpose of an exhaust fan is to eliminate moisture from the bathroom (as well as odors). When you shower, the source of moisture is steam. Steam rises (because it's warm) toward the ceiling. If you can exhaust it before it cools and settles down as moisture on your walls and ceiling, you reduce the chance of mold and damage by moisture in the room. Of course it makes the most sense to have the fan running when you are making steam and positioning it above the source!
