What to do with my Little Lime Hydrangea

I've had my Little Lime hydrangeain this terracotta pot for about a year now. I think it has outgrown it's container so I want to transplant it into my garden. The stems on this plant are rather thin, Maybe a little thinner than a pencil. My question is, when planting should I add some bone meal into the new hole? Also, if I remove the blooms, would that put more energy into growing thicker stems? Should I keep or remove the leaves? Once it rains, they just flop. (See Photos please)
