A Reminder for the Men in your Lives

DH had a routine visit with his internist and the bloodwork included his annual PSA test. It came back at twice the number it was a year ago. Test repeated, same result. MRI reading led to a biopsy the following week. The pathology report is alarming, average Gleason score 9 some samples worse. Prostate cancer is usually a slow moving entity, except when it isnt.


He is going to Florida on Monday for a special PET/CT that uses the prostate specific antigen. There are few locations for that as it was recently approved by the FDA (and not covered by any insurance yet). That scan will tell (30% more accurately than conventional scans) whether and where he has metastases, which the pathology report indicated could be possible. The next day we are going to Baltimore where he will see a specialist at Johns Hopkins. They have the scan technology there, in fact it was invented by one of their doctors, but the appointment for that would mean another week’s wait. We hope we’ll be in Baltimore about two weeks for surgery, then head home for locally administered proton beam radiation. Alternatively, worst case, he might be sent home for a few months of chemo prior to the surgery and radiation.


We are both in a sort of fugue state about this. He has no history of cancer on either side of his family—-literally, they all die of old age, He has NO symptoms. He spent almost two years recovering from a spinal surgery that ultimately resulted in 20 operations and months in the hospital with sepsis. He has recovered enough to live a fairly normal life and now we are facing another battle. Not surprisingly he is feeling like a moving target.


Please make sure your young men have a PSA once just to get a baseline number. And IMO the test should be repeated every six months after age 55. It’s a CHEAP test, costing $20 to $40 in a median range. The problem with an annual test is that an aggressive cancer that grows undetected for a whole year can be stage 4 when it is discovered. What would have been a standard course for a very treatable condition if discovered six months ago is now a very big deal.
