I think there are two issues to consider here. One "technical" and the other "decorative". The first is technical, it has to do with the size of the grout line and the type of grout used. Unsanded grout is for grout lines less than 1/8" wide. Sanded grout is for grout lines 1/8" to 1/2" wide. If the installer exceeded the manufacturer's technical limits of the grout, it's a technical bad on him. Somewhat muddling that issue is the fact the your husband approved of the installation "as is". From the decorative perspective, because your husband approved of this, you really have no recourse. A third perspective that I'll throw out for the heck of it is one of pride. This could have been resolved in a couple of ways during layout. Your installer has no pride.

first is an outside corner - one side will be tile from the shower, and around the corner will be plain drywall. would this work? Yes. Look on the right edge-- you can see it best there: i'm thinking i just need some sort of rounded edge instead of the field tile edge... would this work? i may just leave it field tile since i will be putting tall storage there The rondec will aork there, as well, but if you're going to be building a storage shelving unit, then you can just cut the field tile into the wood.

Press and hold CTRL and press + to inlarge the photo as large as possible. If you look closely the carpet is correct. The tile is correct. The gap between the two is actually fairly even from top to bottom. The photo gives the illusion that the top is much wider. Even this photo shows it very clearly. Honestly to me. It looks like a simple tile layers mistake. To me it appears as if they started at the back wall and worked out toward the carpet. A simple string and two push pins would have prevented the gap altogether. A small string tied to the pin of a push pin and pressed into the outside wall (carpet side) at the door end and stretched tightly to the mirror end outside wall. take the measurements from the string to the back wall (shoes). Then start laying the tile from the carpet back. If a gap shows when you reach the back wall. All trimming would have been less noticeable or hidden by trim, cabinets etc. But to late now. Best thing to do is simply trim (cut to width) the seven pieces and lay them. The transition piece will hide most of it.

Well laid sheets of tile will NEVER have the look of "sheets of tile". The installer is the weakest link in the chain. If this is an insurance claim, then you may have to work with the insurance company's "preferred installers". And that normally means "low-end" quality. The preferred GC's have the lowest bids in the industry (many reputable companies will NOT work with insurance companies because they want the cheapest of the cheap). And real tile setters are expensive. Which means most of these companies will simply use "a guy" to lay the tiles. I would say you are looking at "a guy" type of install. In the world of tile this is not considered acceptable. It would be a redo. My guess is this is the best possible install this guy could manage. Getting him to do it again will probably result in the same issues...which is why you want to ensure the GC is using a tiling professional and not some guy with a trowel. I'm going to guess the GC will be quite indignant should you ask about the qualifications of his crew. Did you know that most insurance companies will offer a "pay out"? This would allow you to find your own company to do the work? I would push hard for a redo. In the building industry the original installer is allowed ONE attempt to correct an issue. Once that one attempt is made (and failed) then you can bring in another professional to do the work...at the original installer's expense. I would check your local/state laws on that one...but you are within your rights to move on should the 'fix' work out to be worse than the original install. Just a little FYI. Do you have any photos of the build BEFORE the finishes were installed? If there is an issue with the quality of the finishes, there are often issues with the quality of the water proofing, etc UNDERNEATH the finishes.
