I think my Japanese Maple is dying.

I have a ~25ft Japanese Maple in my front yard. Over the past couple of years, we've noticed a couple of the primary branches have begun to deteriorate, shedding all of their bark. Totally dead. I was finally able to get some time over the weekend to break out the chainsaw and prune these dead branches off. When I cut the largest of the dead branches (probably 10" in diameter), the core was totally hollow looking into the trunk of the tree. Looking into the branch I had cut, there appeared to be a white mold inside.

It might also be worth mentioning that there was a flat, brown fungus growing perpendicularly from the branch near the position where I cut it. Unfortunately, I didn't get a photo of that.

I'm no arborist so I'm not really sure where to go from here. My wife and I like the tree a lot and would like to save it but I'm concerned it's not salvageable.
