English Ivy care

We have English Ivy as a ground cover in our front and side yards. It has done nicely except for a few bare spots where it seems to have died. We are in the woods, surrounded by oak and hickory trees, and the leaf cover is quite heavy. Grass and other ground covers will not do well. Should I leave the leaves on or rake them off? And also, when should I fertilise? Our soil is very clayey, which keeps the ivy from becoming obstreperous, but keeping it looking lush can be a dilemma.
