Orchid overwatering, help save

Hi, I have an orchid well over a year that was only leaves (deep green, due to lack of lighting I suppose) but it was thriving. I noticed the original potting medium (a mix of moss and bark) was disintegrating and I got rid of the old stuff and added bark. It's not been doing well since and dropped 4-5 leaves and the rest seem to be on their way out. I thought I nicked a live root while cutting dead roots. I also noticed some white mold on the bottom. It's never re-bloomed but it was happily growing before. I don't know if it just doesn't like my new potting medium, or what I can do to curb the leaf loss? Current photo is attached. Thank you!
