What’s wrong with my plant?

I recently transplanted a small ornamental Phalaris grass my friend had divided.

For a couple weeks, it did well, with new green shoots growing. In the month since then, it's done poorly (minimal growth, yellowing/browning blades)

The 3 gallon fabric container medium is Coir:Perlite @ 2:1 ratio. I water with MaxiGro complete/stand alone nutrients at moderate strength, pH'd to 6.0

Suspecting fertilizer burn, I stopped nutrients and flushed with only water for 2 weeks, and the plant deteriorated even further. Re-introducing nutrients gave a short, initial improvement.
