It probably resulted because kumquat doesn't have the highest degree of compatibility on that citrange rootstock. The rate of growth on the citrange is much faster than the kumquat. This can lead to some dislocation.
Just watch it to not develop any mold in it due to water, I had kind of dark mold growing in the area of the failed graft on one of my trees ( not grafted by me) and cleaned it with a Q tip dipped in rubbing alcohol and still inspect it from time to time.
