How to build my own glass shelving for a medicine cabinet

The house I just moved into has a half-finished medicine cabinet. There's already a wooden box built into the wall (Dimensions: 14.5" (w) x 25" (h) x 4 1/8" (d)). However, the cabinet is missing shelves and holes for the support tabs.

Does anyone know of any pre-cut shelving that could fit these dimensions? I've searched the web, but come up empty. Alternatively, anyone know of a place where I can get glass cut relatively inexpensively? Would I need tempered glass for this?
