New toilet flapper doesn't always work

I recently replaced the flapper in our toilet tank with another just like it. Both are "Korky" brand. The old one worked fine for years, but gave way where the chain attached to it. We need the collar to slip over the standpipe because the ears on the ring that the flapper rests on are bent and in the wrong location.

The problem is that the flapper works sometimes but from time to time it will leak, the flat will drop an inch or so, and it will refill; this will continue until we re-flush it or reach into the tank and fuss with it a little.

I replaced the Korky with a Fluidmaster that is similar, and it does exactly the same thing. I did play with the chain length but that doesn't seem to have helped. I tried for about 1/2" to 1" slack. I tried another with a rubber "chain" and had the same problem with it.

Should I just buy all new parts for the inside of the tank, or is there some simple solution I'm overlooking?
