Advice on repotting my Guardian Angel

My Guardian Angel is one of my hostas that hasn't been doing as well as I think it should. So why not have a look at the roots and see what is going on? Last repot was 2013 so it definitely needs it.

The coloring on it is quite nice, especially for this time of the year.

So here are the roots:

I found a few rotted roots. But I would say that the roots are rather short. Probably due to rotting at the ends? I see thick roots one side and and thin roots on the other.

And I see this strange looking elongated crown. I don't know what this means. Should I cut if off?

So my questions are:

If the root is healthy, but then has some rot at its ends, can I just cut off the rotted root part, or should I remove all of this root?

Should I cut off some of this crown?

This GA was one eye when I got it in 2013. And it's still one eye! Help!

I have put it in some water for now.
