Why is my Lemon tree getting burnt leaf tips?

Hey Fellow Citrus Lovers,

I've been having problems with my 3 year old citrus, Lemon tree grown from an organic Lemon from the local Supermarket. I've attached some pictures.

Ever since I moved to my new house (1 1/2 years ago) I've been having problems of the leaves of my lemon tree starting to yellow, and then dying to the point of leaf drop. I've always used Jobes Granular Organic Citrus Fertilizer (3-5-5 NPK) since age 1. I've just recently re-potted due to a fungus gnat problem, and it is finally growing some new growth, but I've noticed that, again in the past month that the leaves on the tree for a second time this year has started to burn/yellow at the tips! Can someone please tell me what I am doing wrong, I've already lost one of my trees to the fungus gnats and root rot (still trying to save it though) and this is my last one. Any comments would be very much so appreciated. (Sorry for such massive pictures)
