Small plants with white clusters

These small plants were in disturbed, shaded ground in a state park this morning. They were 4-6 inches tall, with 4 to 6+ leaves emerging from each single, cupped stipule at 3 or 4 sites along the stem. The leaves looked like they were in whorls, but were actually in two opposed bunches from each stipule. The tiny flowers were in terminal clusters, and had either 3 stamens and 3 petals, or 4 stamens and 4 petals, sometimes with both types in the same cluster (see second photo). The flowering heads were only about 1/4 inch across, and the flower bases were tubular.
