Installing Hardwood over hardwood, and building up the other areas

We're preparing a kitchen remodel, and while doing so, we want to get new hardwood floors installed in the entire main living floor for a continuous flow.

Currently the kitchen, dining and entrance/hallway (3/4 of the floor) are covered in solid 2 1/4" wide 3/4 in thick oak floors. I want to put 5 inch handscraped plank floor in.

Rather than tearing out the existing floors, we're going to install the new floor over the existing hardwood.

Problem: The living room (the remaining 1/4 of the floor) is carpet, and therefore once removed, not at the same height as the remaining floor.

What is the best way to "build up" the living room floor so that it is the same height as the kitchen/dining/entrance so that the wood can be installed seamlessly throughout?
