window cuts in concrete foundation pour question

Hello, I had a quick question regarding a 9' poured concrete foundation. We plan to have three large egress windows (4'x4') in window wells and three hopper windows (3' x 2').

Excuse my ignorance but are the window holes cut out after the pour or are the forms set up so that the window cuts are made during the pour. I assume that these "holes" are three sided with the top part covered with a 2x for the header? If this is the case I assume the window headers would be set at the top of the foundation, even with the two top plates.

How is the window header height specified on the drawing. The architectural drawings Ive seen do not specify the header height but only specify the max sill height (44") for code.
