Wiring a 4 wire hot tub to a 3 wire circuit

I am replacing an old hot tub with a new, 240v tub that expects four wires. The GFCI disconnect I bought also expects four wires. The old tub used 3 wires (two hot and a neutral). Tracing the wiring back, the neutral is connected to ground at the main panel (as are all the other neutrals).

So, how do I connect this 4 wire GFCI (and tub) to a panel that only offers three places to connect (the two hots and the combined neutral/ground)?

I could run a new, separate ground wire back to the panel, but it wouldn't be doing anything different that the existing neutral, so I might as well just combine neutral and ground at the GFCI.

Or, I could just leave ground disconnected at the GFCI.
