Paint on doors is cracking/peeling

I tried doing a search to see if this has been addressed, but couldn't find anything.

I am hoping that someone can help me. When we first moved into our home a few years back, we painted over all of the trim in the house. It was cream (or maybe the paint had yellowed?) and we wanted white. We cleaned, sanded, primed and painted two coats of Benjamin Moore semi-gloss. After just a couple years the paint in most of the edges of the panels (they're 6 panel doors) is cracking and peeling. My question is in two parts:

1. what did we do wrong? Why is this happening?

2. How do we fix it? Can we do it without having to sand it off. We have an infant and I'm worried about creating lead paint dust.

Is this something we can do ourselves or should I just suck it up and spend money (that we don't really have) on a pro?
