Hoarders

I know there was a thread about this before, but I just started watching this series last week. I am so confused about hoarding, and I am trying to understand it. For example, how can some of the people say one minute that they didn't know they had a problem, and then the next minute say they hadn't had anybody in their home for a year because they were embarrassed? And how can some people say things that are obviously not true- eg. pointing to a book and saying they have to keep it because they were reading it just last week when it's full of cobwebs and obviously hasn't been touched in ages? I know the quick answer is that they are in denial, but what's the difference between denial and lying?

I know it's a mental illness, and I have a "handle" on all different types of mental illnesses, but I have trouble grasping this one. Oh, and I find it interesting that obsessive-compulsive can manifest itself in some people by making them wash their hands obsessively and do things like that, or it can manifest itself in others by making them collect trash and garbage and rotten food.

How does having the urge to collect so much stuff because you have emotional attachment to it, mean you then throw it all over the floor? Why are you abusing stuff you can't bring yourself to get rid of because it has sentimental value to you?

I also don't understand how people who very much love their children can have them live in those conditions. I feel so self conscious asking these questions because I kind of feel inadequate that I don't get it. I do feel compassion for them, but I also feel confused, intrigued and kind of disgusted. I heard a doctor today say "they aren't slobs" and even THAT comment confused me. I would have thought that although it's such a derogatory term, they kind of are, but that it's caused by a mental illness. I have no idea what I'm missing...
