You could try saving the original as a template (.dot extension) and see if that works.

I don't use word but still have word 97 on my computer. I tried saving as a template and got the same resuslt as ess1 so I did a word search for 'create form' and found there is more than one type you can create in word. I think the type you want to create is the 'online form'. Here are the instructions from word 97 but you should probably do the same search in your version:

This is funny because I spent hours trying to do the same thing the other night. I downloaded the template, but when you try to put the info in, it moves the information around. I spent so much time on this I finally gave up.

All this stuff just takes so much time and is so 'counter-intuitive.'

When creating a form document, after you type what you want, click "ab" on the form template. This will create a form field. You can add these anywhere in your document. If you want to add a check box, just click the box with the check mark on it. After your form is complete, you have to click on the "lock". When you do that, the only places you can type in are the form fields you created. You have to use your tab key to move from field to field. If you need to add another form field just click the "lock" again and your document will be unlocked. Clicking the "a" will highlight your form fields.
