How weird is it to see a toilet right when you walk in a bathroom

We are in the process of designing our kid's bathrooms. All are pretty much a rectangle 7 X 9' size. Right now the door is in the center opening outward and you see the toilet straight on against the back wall (widthwise) with the tub and vanity/cabinets on either side going lengthwise. This allows the vanity to be a good 5 1/2' long.

Would you recommend placing the door to the side, moving the vanity against the same back wall next to the toilet. This would shrink the vanity to be about 3' and inches wide.

Also, side note, do most people have the bathroom doors opening to the inside or swinging out?
