I know Tripple Brook Farm, a mail order nursery in western Mass. sells it . As to whether they are a 'good' nursery, the reviews are mixed (check out Gardenwatchdog.com), but they do have lots of hard-to-find natives.

The best looking plants that I have ever got came from
    Arrowhead Alpines

Are you going to be growing these in the ground? In pots?
I really would appreciate knowing how you are going to
handle these native orchids. Reason why I am working on
developing an appropriate pot culture for them, under the
conditions that I have to work with, and am always on the outlook for additional insights that will aide me in the
successful cultivation of these great little orchids.
Currently the ones that have survived my cultural experiments are in my greenhouse, mostly in my debris mix though I am also using straight cypress mulch as an experiment. I have two plants in spike and 2 or 3 that are putting out side growths!!. The measuring rod I am using is
the condition of the roots and at some point I will unpot them to check out just how the roots are responding to my cultural experiment. So if you have any suggestions I am all ears!!!
