Hydrangea tree or shrub??

This plant was grown from a cutting from my mother-in-law, who doesn't remember anything about the original plant. It's about 5 years old and has flowered for the last 3, but only had 3 or 4 blooms each time. The blooms are dome-shaped and greenish-white (I'm in zone 8 with very acidic soil). I'm pretty sure it's a hydrangea tree, but don't know if I'm supposed to prune it in order for the plant to establish a trunk; right now it's just a shrub and getting larger every year! I've got some links below that show the whole plant, a couple of close-ups and a close-up of the trunk underneath all those branches.

Any details about what type hydrangea it is and how to best care for it are much appreciated!
