How many people actually perform a soil test?

I never tested my soil, I guess because I figured if it grows and it's healthy, my soil must be fine. If it doesn't grow (perennials, for the most part), I'm not buying it again because it's just not happy here.

Aside from an acidic fertilizer on my acid loving plants each spring, the only other annual amendment is a layer of composted manure to my garden beds, early spring. I have clay-ish soil, but lots of nightcrawlers, which I've been told is a good sign.

When is it actually necessary to perform a soil test? Do you do it? If so, how often?

Does planting trees deplete the soil of nutrients in the long run? Neighbors have a row of trees opposite my fence.
