Soil Test/Analysis Results - Need advice please!

Live on ~2.5 acres composed of heavy clay soil in Central Ohio. Lawn is a ryegrass mix (Double Eagle) & is about 3 years old. It is doing 'okay', but still has many smaller bare spots, thin areas and it's fair share of weeds/crabgrass (we built in the middle of a rye/hay field).

Now, my question - what to do with my rye grass mix in heavy clay soil?

Looks like i need lime. What's the 'dolomitic/calcitic' difference? Should i use pelletized or pulverized? can i put it on anytime? How do i get the nitrogen? Is a natural means cheaper/better? I think i need potash but i CANNOT seem to find it anywhere in bulk for a 2.5 acre lawn - any suggestions? Can i put potash down anytime? Can I mix everything together and broadcast it all at once?

I would love some advice as to what those 'in the know' would do at this point - and thanks in advance!!!
