blueberries slow death, whats going on?

Last year I posted some pic's of my blueberry plant when it started to show signs of some sort of disease/issue. I pruned away the dead or dying branch's but its back again. Can someone help me diagnose what's going on. I have another plant right next to it that is doing fine. Only certain parts of the plant/branches seem to have the dried/withering leaves. After the leaves wither up the branch essentially goes dead. With all the dead branch last year I had to prune quite a bit. The plant did not have one blossom this year. We have had ample rain this year. I have applied the soil acidifier twice this year since March. Miracid twice since May. Same as the adjacent plant. I do have both chipmunks and voles that have tunneled under the plant. Once again , same as the adjacent plant. I just bought a new Patriot plant. Should I just dig up the "problem" plant and plant the Patriot? Any ideas?
