Can we save a 100 year old apple tree

My next door neighbor has a beautiful old apple tree in her front yard. It's easily 100+ years old and may be the last remaining tree from the working orchard that was located here back in the 1800's. Due to disease and such the tree has gotten very side heavy and is slowly tipping over.
Any thoughts on the chances of saving this beauty through some judicious, albiet heavy, pruning? I couldn't post pix here so they are in the ID forum. (I hope that isn't breaking any of the rules.)
